PPTA AWARDS
SELECTION CRITERIA & GUIDELINES

CARLIN-MICHELS ACHIEVEMENT AWARD

Criteria

  1. The recipient must be a physical therapist or physical therapist assistant who is a member in good standing in the PPTA/APTA at the time of selection. Members of the Awards Committee are not eligible for the Carlin-Michels Achievement Award.
  2. The recipient must have been a member of the Pennsylvania Chapter for five consecutive years prior to the nomination of the award.
  3. The individual must have made an outstanding contribution to the profession of physical therapy, to the American Physical Therapy Association or its components and/or to the community.Professional achievement may be demonstrated in one or more of the following areas:
    1. Patient care through direct service, consultation supervision, or administration.
    2. Education through teaching, directing continuing education programs, developing or implementing teaching methods, or administering physical therapy programs.
    3. Research in professionally related areas.
    4. Service to the community through professional health related organizations, private or governmental agencies.
    5. Service to the APTA or its components through promotion of the organization and its goals, service in elected offices or service on committees and task forces.
  4. The individual must have shown a deep concern and active interest in the development and growth of the physical therapy profession and have served as a model for other professionals.
  5. The award will be presented to a physical therapist or a physical therapist assistant who has continuously demonstrated and maintained high personal and professional standards of conduct.

Guidelines

  1. The recipient of this award will be chosen by the Awards Committee. The Chairman of this selection committee will be the Member-at-Large.
  2. The Awards Committee shall function from the close of the Fall Board Meeting until the presentation of the award at the Annual Conference the following year.
  3. Each committee member will request nominations from the general membership. Letters of nomination must be drafted for each nominee, stating the reasons for selection. Resumes must be submitted for each nominee.
  4. Letters of nomination must be forwarded to the Chapter Office by June 30th. The letters of nominations and resumes will be forwarded to all members of the Awards Committee.
  5. The Awards Committee Chairman will verify that the nominee meets the membership criteria for the award prior to the Awards Committee Award Recipient Selection Meeting.
  6. The Awards Committee Chairman and committee members will confer on all letters of nomination at the Selection Meeting.
  7. A majority vote of the committee members will be required for selection of an award recipient.
  8. The Awards Committee Chairman will notify the Chapter President if a recipient was selected within one week of the committee meeting.
  9. The award will be presented to the recipient by the Awards Committee Chairman during Annual Conference.
  10. A newsletter article will be published annually by staff to achieve the following goals:
    1. Educate the membership about the awards and the selection criteria.
    2. Publish the deadline, June 30th, for submission of names to the Executive Director.
    3. Publish the names of past recipients of each award, highlighting those announced at the prior PPTA conference to be included in the first post conference issue of the Newsletter.

Please complete the PPTA Award Nominations Form to be considered. A current resume/CV and a letter of recommendation are required to submit your nomination. Each year the nomination window opens in the Spring and the deadline for receipt of nominations is sometime in July (specific dates TBA).

SERVICE AWARD

Criteria

  1. The award will honor service to the PPTA by an individual in a non-elected position, or for service by an elected official within or outside the scope of their responsibilities of office. Members of the Awards Committee are not eligible for the Service Award.
  2. The award may be presented to a physical therapist or physical therapist assistant who has significantly contributed to the betterment of physical therapy within the Commonwealth of Pennsylvania.
  3. Selection may represent either intensive short duration service or consistent service of long duration as a supporter of the Association and its goals and objectives.
  4. The recipient must be a member of the PPTA/APTA.
  5. The award should reflect service to the Pennsylvania Physical Therapy Association.

Guidelines

  1. The selection of a recipient of the Service Award shall be the responsibility of the PPTA Awards Committee.
  2. A newsletter article will be published at least yearly by staff to achieve the following goals:
    1. • Educate the membership about the awards and the selection criteria.
    2. • Publish the deadline, June 30th, for submission of names to the Executive Director.
    3. • Publish the names of past recipients of each award, highlighting those announced at the prior PPTA conference to be included in the first post conference issue of the Newsletter.
  3. The Executive Director shall be responsible for discreetly compiling a dossier which hopefully would include a curriculum vitae of the nominee. The individual making the nomination can be requested to assist in this process.
  4. The two-hour unopposed meeting time will be planned for the Awards Committee at a Go To Meeting.
  5. The Service Award will be presented during the PPTA Annual Conference.

Please complete the PPTA Award Nominations Form to be considered. A current resume/CV and a letter of recommendation are required to submit your nomination. Each year the nomination window opens in the Spring and the deadline for receipt of nominations is sometime in July (specific dates TBA).

HUMANITARIAN AWARD

Criteria

  1. The nominee must be a member of the PPTA/APTA in good standing for at least one year.
  2. Service, which must be outside the profession of physical therapy, may be of intensive short or consistent long duration.
  3. Nominations will be accepted from the PPTA membership. Nominations must be supported via letters of recommendation and/or nomination from the PPTA membership AND from organizations and/or individuals outside the profession of Physical Therapy.

Guidelines

  1. Nominations must be submitted to the Executive Director by June 30th . A call for nominations will be placed in the Chapter newsletter along with other calls for awards nominations. The Executive Director shall be responsible for discreetly compiling a dossier which should include a curriculum vitae of the nominee.
  2. Selection will be made by the Awards Committee.
  3. Elected members of the PPTA Board of Directors are not eligible for nomination.
  4. The award will be conferred at the PPTA Annual Conference. 
  5. The Executive Director will arrange for an article to appear in the first postconference issue of the Newsletter which will explain the award, highlight the current recipient, and publish the names of past recipients.

J. KENT CULLEY FELLOWSHIP AWARD

Criteria

  1. The award will honor service to the PPTA by a non-physical therapist/nonphysical therapist assistant who has significantly contributed to the betterment of physical therapy within the Commonwealth of Pennsylvania.
  2. Selection may represent either intensive short duration service or consistent service of long duration as a supporter of the Association and its goals and objectives.
  3. The award should reflect service to the Physical Therapy profession within the Commonwealth of Pennsylvania.

Guidelines

  1. The selection of a recipient of the Fellowship Award shall be the responsibility of the PPTA Awards Committee.
  2. A newsletter article will be published at least yearly by staff to achieve the following goals:
    1. Educate the membership about the awards and the selection criteria.
    2. Publish the deadline, June 30th, for submission of names to the Executive Director.
    3. Publish the names of past recipients of each award, highlighting those announced at the prior PPTA conference to be included in the first postconference issue of the Newsletter.

STEVEN KOLUMBAN AWARD

Purpose
To acknowledge a Physical Therapist Assistant member of the PPTA who has demonstrated outstanding achievement in clinical practice, community service, and personal commitment to Physical Therapy.

Eligibility

  • A nominee for the award must be a member of the Pennsylvania Physical Therapy Association
  • A member in good standing of the PTA Special Interest Group for two (2) years.
  • An individual may receive the award only once in a three (3) year period.
  • Members of the PTA SIG Executive Committee shall not be eligible for the award during their respective terms of service.

Criteria for Selection

  1. Outstanding achievements in the practice of Physical Therapy and service to the Association are demonstrated by:
  2. Professional contributions made to the PTA Special Interest Group, the American Physical Therapy Association, and the Pennsylvania Physical Therapy Association.
  3. Community services and activities that enhance the quality of life and function.
  4. Exemplifies and promotes the role of the Physical Therapist Assistant through education, practice, or research.
  5. Research involvement/activities, publications.
  6. Promotion of ethical standards and professional conduct among peers, patients, and students.
  7. Professional development through continuing education; courses, workshops, inservices, etc.
  8. Ability to inspire patients, peers, or students to perform at, or strive for achieving optimal potential—to be the best that they can be.

Procedure for Nomination

  1. Nominations for the award may be submitted by any current member of the Association.
  2. The Nomination Letter should include reason for selection.
  3. Please complete the PPTA Award Nominations form to be considered. A current resume/CV and a letter of recommendation are required to submit your nomination. 
  4. Each year the nomination window opens in the Spring and the deadline for receipt of nominations is sometime in July (specific dates TBA).

Presentation

  1. The award will be presented during the Annual Conference of the PPTA. Selection for the award is not contingent upon attendance at the Banquet.
  2. The award recipient will receive a plaque and recognition at the Annual Conference.
  3. A permanent record of the recipients of the award will be kept by the PTA SIG Secretary on the PPTA Webpage.
  4. A submission for the Chapter newsletter will be made by the PTA SIG Chair to indicate the award recipient and some information of the reason for nomination and information about the award.

PPTA STUDENT AWARDS SELECTION CRITERIA & GUIDELINES 

STUDENT LEADERSHIP AWARD 

A review panel will review the applicants, and final review by the membership chair will determine the recipient(s) of the Student Leadership Award based on the criterion of: 

  • APTA involvement
  • Leadership
  • Community Involvement 

A framed certificate will be presented during the annual membership meeting at the PPTA conference in the Fall for the student recipient(s) and those schools of the student recipient(s), as well as being awarded with a leadership pin and first year APTA career starter dues fully paid by the PPTA membership committee (state dues to be paid by Student Leadership Award winner). 

Requirements for all applicants: 

  • Must be a current APTA member and a student in a Pennsylvania PT, PTA, or transitional PT program anytime within the calendar year.
  • Must complete the online application.
  • Must attach either letter of recommendation from a non-student APTA member (i.e. Clinical instructor) to this application or have the sponsor email letter of support to Lauren Bilski - please list the student's name as the email subject.  
  • Each year the nomination window opens in the Spring and the deadline for receipt of nominations is sometime in July (specific dates TBA).

DR. TOM STEWART AWARD

This award is intended to provide an opportunity for a student to become more involved in the professional association. The award provides funding for PPTA annual conference registration, hotel expenses, and admission to the PAC event, to a PT student who will be in their 3rd year at the time of the conference. 

A panel will review the applicants, and final review by the membership chair will determine the recipient of the Dr. Tom Stewart Award. 

Requirements for all applicants: 

  • Must be a current APTA member and a student in a Pennsylvania PT, PTA, or transitional PT program anytime within the calendar year.
  • Must complete the online application
  • Must provide a 1-2 paragraph statement on future PPTA involvement. 
  • Each year the nomination window opens in the Spring and the deadline for receipt of nominations is sometime in July (specific dates TBA).

PPTA SPECIAL INTEREST GROUP (SIG) AWARDS SELECTION CRITERIA & GUIDELINES 

The Awards Committee for PPTA Special Interest Groups annually seeks nominations for the Acute Care, Geriatric, Pediatric, Neurology, and Private Practice SIG Awards. The awards are presented at the PPTA Conference each year. 

The awards for each SIG recognize a physical therapist or physical therapist assistant that is a member of APTA/PPTA and has made meaningful contributions to physical therapy at the individual, chapter, or national level in the practice area supported by the SIG. The contributions can be in any of the following areas: 

  • Excellence in patient care or clinical practice
  • Excellence in education or research 
  • Community advocacy 

If you would like to recognize a colleague for excellence in acute care, geriatrics, pediatrics, neurology, or private practice, please submit a letter of nomination digitally, or mail hard copy submissions to: PPTA Chapter Office, SIG Award Submission, attn. Allison McIntosh, 2400 Ardmore Blvd., Ste. 302, Pittsburgh, PA 15221. Digital submission of nominations is highly encouraged. The nomination should provide evidence with specific examples of the individual’s contributions. A current copy of the nominee’s curriculum vitae or resume is preferred but not mandatory.
